physicians will be happy to complete necessary forms. It is
possible that a $15.00 fee will apply in certain instances.
Medical records may be transferred by filling out a medical release
form. If you should choose to obtain a copy of your medical
records yourself, there will be a $30.00 fee.
You can download necessary
forms from this site and mail or fax them to our office before your
appointment.
